Even if at the moment a standard notion of Disease Management doesn’t exist, since the concept is relatively new and still in an evolution phase, it is possible to summarize it as a systematical approach to the global management of a disease, through the application of integrating prevention and treatment interventions.

Basic elements of the solution are:
- the clinical or surgical department;
- services;
- public access points;
- internal communication system;
- management and administration services.
All these elements are connected and clinical information (requests, reporting, and any other clinical datum) and non-clinical information (any kind of document) can be exchanged in a direct and fast way.

Many computerized medical records have been produced. Some are too operational and a little clinical, others are much oriented towards decision-making, but a little efficient in clinical data management, others are still too “to measure” for a specific kind of physician (usually, the General Practitioner).
All of them have two common characteristics: they are not linked to the operational context and they are built not on the patient, but on the operator.
